Do you recognise a patient struggling to manage their medication or struggling to remember to take them on time.
You can refer them for an assessment to the adult social care team at Manchester City Council. We will talk to them and understand how they are currently managing and assess and advise on their needs. This assessment is free of charge for all, but there maybe a charge for the service if the individual doesn’t qualify for social care.
They need to be:
- Over 18 years of age
- Live within Manchester City Council area
