Refer a Service User to YOURmeds | For Social Workers

For social workers & care coordinators

Protect your service user. Take a visit off your caseload.

YOURmeds is a NICE-recognised medication monitoring system that pharmacies fill and post out weekly. It prompts the right dose at the right time, flags a missed or incorrect dose the moment it happens, and gives you real, reviewable evidence for your care plan — without adding a daily visit to anyone’s diary.

Happy lady managing her prescription with yourmeds

Backed by real-world validation

Real-world validation, not just a good pitch

YOURmeds has been trialled and evaluated by university health boards and councils across the UK. Including Welsh Government-backed innovation funding, an NHS Wales Award shortlisting, and published local authority case studies.

Bridgend / Cwm Taf Morgannwg University

Backed by Welsh Government SBRI innovation funding — an 82% improvement in medication adherence and a 50% reduction in unnecessary carer visits, with real-time alerts giving families confidence around the clock.

Leeds City Council

Across 15 people over 81 days, roughly 1,676 medication visits were saved, averaging a 6.55:1 return on the cost of the service.

Stoke-on-Trent City Council

Nearly 2,000 care hours saved across 20 people, a 4x return — now commissioned for 150 people across the city.

Figures and outcomes are drawn from each organisation’s published case studies and reports.

Who this is for

If medication is the safeguarding risk on your caseload, this is worth a look

Under the Care Act 2014, you’re already weighing wellbeing, independence and safeguarding for these exact situations.

Living alone, at risk of missed doses

Older adults or anyone managing multiple medications without daily support nearby.

Memory difficulties or dementia

People who forget whether they've taken a dose, or take one twice, without realising.

Recently discharged from hospital

New or changed medication regimes that need extra support to bed in safely at home.

Learning disabilities

Support to build independence with medication, with a safety net for family or carers.

Mental health support needs

Consistent prompting for anyone whose routine benefits from an external nudge.

Family carers who can't be there daily

Peace of mind for relatives who worry when they can't check in person.

How YOURmeds helps

Evidence for your care plan, not just a reminder

The pharmacy fills it, YOURmeds prompts it, and you get the data — without anyone needing to visit just to hand over a tablet.

1. Pharmacy-filled, ready to use

Medication arrives pre-loaded into numbered trays, posted out weekly or monthly — no one has to fill a dosette box by hand.

2. Prompts at the right time, detects the wrong one

A number appears on screen when it's due. Sensors can tell if the correct dose, the wrong dose, or too much was taken — flagging a real safeguarding risk, not just a missed reminder.

3. Supporters are told automatically

Up to 5 nominated people get a free app showing what's been taken and what's due, with an alert the moment something's missed.

4. You get reviewable data

Real adherence data you can point to in a review or funding panel, instead of relying on a good day at a home visit.

What happens after you refer

A referral shouldn't disappear into a queue

Here’s the actual turnaround you can expect once your form lands with us.

Day 1 — Referral processed

We confirm receipt straight away and process your referral within 1 business day. If anything's missing, we'll tell you exactly what, rather than letting it stall.

Day 2 — Pharmacy set up

If you consent to switching to a YOURmeds partner pharmacy on the form, this is faster — we confirm the service user's start date and get equipment posted out within 1 business day. Keeping their existing pharmacy may take a little longer.

Day 3 — Medication delivered

The pharmacy delivers the first medication to the service user. Outside weekends, the whole process typically takes around 3 days start to finish.

How to refer

Takes less time than one home visit

No portal to register for, no phone queue. Just the form.

Step 1 — Download the referral form

One page, straightforward, and quick to complete alongside your usual assessment notes.

Step 2 —Complete the form

Add the service user's details, most of this you'll already have from your assessment. You'll also need the service user's medication timings, so it's worth having their MAR chart or care plan nearby.

Step 3 — Email it to our referrals team

Send the completed form to referrals@yourmeds.net and we'll take it from there.

Referring costs nothing and takes a few minutes. Funding for the service is arranged directly with your local authority on our end — if there isn’t an existing arrangement in place yet, that’s a conversation we have with your council, not something you need to sort out.

Common questions

Questions social workers ask us

Referring costs nothing. Ongoing use is often fundable through a Personal Budget or Access to Work, and our team can help complete that paperwork rather than leaving you to navigate it alone.
YOURmeds is recognised in a NICE Medtech Innovation Briefing (MIB289) as the first digital medication system with real-time adherence monitoring. Councils including West Lothian, Leeds and Stoke-on-Trent have published real-world results — details are above. NICE is clear this evidence is promising but still early-stage, not a large clinical trial.

YOURmeds still helps. The device itself prompts and reminds at the right times regardless. Where no supporter is available, a monitoring-centre option can respond to missed doses instead of a nominated family member.

It’s designed for standard oral tablets and capsules. It isn’t currently suitable for liquids, inhalers or injectable medication — worth checking the service user’s regime before referring.
We confirm receipt immediately and process referrals within 1 business day. Pharmacy set-up and equipment dispatch typically follow within another business day, with medication delivered to the service user around day 3, outside weekends.
The device alerts repeatedly until it’s accessed, and nominated supporters are notified through the app. Sensors can also detect an incorrect or extra dose, which we treat as a genuine safeguarding signal, not just a missed reminder.
